Flu vaccination information for parents and carers: please complete online consent form before November 9th
All primary-age children from Reception to Year 6 are entitled to an annual flu vaccination in school. This vaccination is free and is a quick and simple spray up the nose. Even if your child had it last year, it is recommended to have the flu vaccine again this year.
This year, the School Age Immunisation Team in Stafford is moving to electronic consents for the flu vaccination programme. In the event that this is not suitable, a paper copy of the consent form can be obtained from the school office - please contact us.
Please read the letter below for further details, and for how to notify the Immunisation Team whether you consent for your child to receive the vaccination in school:
To indicate whether you consent to the vaccination, as per the letter, please visit https://forms.mpft.nhs.uk/. You will need the following information to complete the form:
A unique code which identifies our school (for data protection purposes, this will be texted separately to your nominated mobile phone number).
Your child’s name, school and Date of Birth
(optionally) your child’s NHS number
Vaccinations will be given to children in school on Tuesday 10th November, so please complete the online notification by Monday 9th November at the latest.
If your child has a long-term medical condition they can also access this vaccine at your GP surgery, and for those children that cannot have the nasal spray, they should have the injected flu vaccine instead.
If there are / have been any changes to your child’s health in the 14 days before the vaccination date you must contact the immunisation team on 0300 124 0366, as your child may not be well enough to receive the vaccination. This would be appropriate, for example, if:
Your child is asthmatic and becomes wheezy
Has required oral steroids
Has increased their asthma medication
